申请垫付准备资料
请尽快准备并提供以下资料：
1、垫付申请及委托书：请下载打印，参考样本完成填写后，本人手写签字，请老师盖章。
Application Form for advance payment: please download and print this form. After filling in the reference sample, insurer is supposed to sign his or her name by hand and ask the teacher to seal it.

2、有效期内的护照首页拍照 Photos of the passport within the validity period.
3、完成之后，以上两个文件请拍照或扫描清晰图片回复至此邮箱：rycy@lhgj.net
After completion, please take photos or scan clear pictures of the above two documents and reply to this email: rycy@lhgj.net
注 Note：
（1）请前往公立医院，普通部门，尽快找到主治医生给我司来电，我司需要跟医生了解您的病情，判责是否符合垫付条件。
Please go to public hospitals and general departments to find the attending doctor to call us as soon as possible. We need to know your condition with the doctor, and check whether the responsibility meets the advance payment conditions.
（2）病房要求：3人或3人间以上普通病房。单间、包间或者VIP房间无法使用垫付服务。伙食费、陪床费需被保险人自己承担。
Ward requirements: 3 or more ordinary wards. Advance payment service is not available for single room, private room or VIP room. Meals and bed expenses shall be borne by the insured.
（3）请使用英文护照名办理住院。如果办理住院是名字过长，请使用完整且正确的姓氏或者名字部分即可。
Please use the English passport name for hospitalization. If the first name is too long, please use the complete and correct last name or first name part.
（4）请办理好住院手续后收集护士站电话提供给保险公司。
Please collect the number of nurse station and provide it to the insurance company.
（5）请保留好所有单据（包括押金条）并接到出院通知后，请即刻联系我司，以便我司跟进出院事宜。
Please keep all the documents (including the deposit slip) and contact us immediately after receiving the discharge notice so that we can follow up the matters of entering and leaving the hospital.
